  1. Recently, Duc Mai (the customer) was filing his tax returns. He noticed on his medical expense summary a few things that he did not consent:

    1. A prescription medicine that he did not request from Walmart. It seems that the person that prescribed it was an employee of Walmart. The only employee that handles the records would be Brian Bao-Kha Nguyen Lic# 48120.

    2. Upon the unknown prescribed medicine, he forged the doctor's (Vo Phac Van) signature for the prescription in order for him to fraud the order without Duc Mai's acknowledgment.

    3. Brain Bao-Kha Nguyen also, fraud the insurance company. Which was reported as well.

As part of this community, I do not condone a person to practice medicine without having the accountability or suitable to assist the people that needs help. To abuse his license for his own benefits. While our identity/information is NOT secured in Walmart's database. I fear for the community of having a dishonest Manager employee of Walmart. Please look into further details to protect the people that seeks for help from pharmacists.

  5. I lost my lower left leg in a MVA , but I was wondering if it would be possible to get some wider doorways in the restrooms .. the ones with no doors in the front of the store ,, they are not wide enough to fit one of your elec. wheelchairs in..they will not go around the corners . with the basket on the front just won't fit . went to the rear of the store . they would 'nt even try to get in there , hallway too small to get the chair turned enough to get through the door ,, would be great if you all could look into this .. sure would help us who are disabled to shop in the Walmart store in comfort .. thank you ,,,,
